Needing advice on medical evidence

My Doctor has done me a letter of support I wonder if you could advise me if this is ok.
All Personal details and diagnosis info at the start of the letter, then.
At the current time he is being treated with medication to help with the pain, he is also on buprenorphine patches and tramadol, gabapentin. He is also on amitriptyline which To can help with pain control but it Also it is also to try to help with his sleep as he suffers with significant depression.
He struggles to get around by himself due to his physical mobility problems he also cannot use a manual wheelchair, and often stays with his family so that they can help him as he is unable to cope himself and do his own cooking, cleaning and shopping. He has also been tying to move nearer to his family for more support.
Because his physical mobility is severely restricted to very sort distances and gets very breathless , due to his atsma (copd). Which he has inhalers.
He remains under the follow up of a consultant psychiatrist for his depression.
He has suicidal ideation and made several attempts to take his life for which I remain concerned.
As well as the medication I already mentioned previously he is on mirtazapine and diazepam due to his mental health problems.

End of letter is this what they need I'm clueless.
Please help I can't cope much more from the DWP.
Can I use this letter for ESA and when I have to transfer to PIP from DLA.
I was formely known as (GoingOffMyHeadWithThisGovernment) Won PIP November 2017 ongoing award HR both. Now kept and got my ESA Support group but took a while.

The letter looks fine for ESA, slightly less so for PIP, although he says "his physical mobility is severely restricted to very short distances", he does not actually define a distance and PIP is all about the distance.
