I'm really sorry to bother you. I have managed to lose the envelope so send my ESA form to. I live in Shropshire- could anyone let me know where to send it?

Thank you. :)

My Dad phoned the Health Assessment Centre for me so we got the address and got it sent 'Guaranteed delivery' this morning.

I'm glad it's over and now I just need to wait to see if I need to appeal or not!
