Hi please could you tell me how is best to put the information on the forms, I have added additional information and done it as you say, NI number, Name etc, but I am concerned that I have perhaps put too much information and maybe should have bullet pointed it so as it is easier to read.
Should I perhaps put maybe an index with bullet points on for each question/page so it's easier for the people reading it.

There's no right and wrong way to complete the form and there's no right or wrong amount of information to include :)

Personally I would include an index, the easier you make it for those looking at the information to find it when they need to.

If you have the time to move the information around then I think about breaking it up as follows;

- Activity A
- problem 1
- summary of problem 1
- detailed explanation of problem 1
- problem 2
- summary of problem 2
- detailed explanation of problem 2
...

- Activity B
- summary of problem 1
- detailed explanation of problem 1
...

By including a short summary of each of your problems and then a more detailed explanation you are making it easier for those reviewing the document to understand how you are affected.
