Would a carer be permitted to take notes while the assessment is going on? My carer has suggested recording my session, but there isn't enough time for me to arrange this as my appointment is in a weeks time. But because my carer has shorthand skills, I didn't know if she could take notes about questions & my responses.

Yes it is perfectly OK for them to do this, see 2.7.12 of the DWP PIP Assessment guide, there's a copy in the PIP area.
