Hi, In the next couple of days I have to send back a ESA re-assessment form. Should I include old documents supporting my disability, as I did not send it last time and they are out of date? Also, I am no longer on medication as I have found it difficult to remain on it, will this affect my assessment?

There should be no problem with you sending old reports providing you also explain why they are still relevant to your conditions as they are now.
You need to explain why you are no longer taking any medicine, providing you can show that the decision is reasonable then it should not effect your assessment.
