DLA to PIP now ESA50 form was created by swanlake
Hello
I have my DLA to PIP reassessment next week, having been invited early to change over in March I think it was. My DLA award was until Dec 2018 so I'm really unlucky.
I've had to move around a lot over the last two years (homelessness) and I received a letter from ATOS saying they'd asked one or more of the medical people I listed for further details of my condition as I didn't include anything current. The problem is whoever sent information about me from the medical practice didn't really know me then (April) as I only started re-engaging in February this year as I was in so many different places.
Now today I've received a new ESA50 to complete as well and I'm wondering if it's because of what's been written about me when they didn't know me. It's seems too much of a coincidence and everything's happening all at once.
My therapist wrote a letter for me last week explaining about my condition ready for my PIP assessment. He's given me three copies. Shall I give one to ATOS at the PIP assessment and then send one to the DWP also? Who do I send it to? I'm thinking that maybe the DWP read something unhelpful in any medical report sent to them that triggered the ESA 50. Is there any way of obtaining a copy of that, what would that form be?
I have mental health issues and am now waiting for a referral to specialist services, but that diagnosis hadn't been decided upon before. Hope that makes sense, I don't want to give too much away. Just scared of course. Thank you for any advice.

It will probably be easier to track this mysterious report from the GP/doctor end than the other. I assume you have kept a copy of your PIP form, so contact the people you listed in it to see if they were contacted by ATOS and If they were to get a copy of the letter or report that they produced.

Thanks very much for your reply Gordon.
I tried to find out today but can't get an appointment until next Thursday; my therapist is away on holiday for the next two weeks otherwise he could access my notes. Aargh lol
So I think I'm going to have to bite the bullet and give my therapis's letter to the PIP assessor and also send a copy to the DWP just in case it isn't forwarded after the medical assessment. The information in that letter is important, I ust hope it's not contradictory (if indeed anything was submitted).
Gordon, am I being foolhardy? I fear without the therapist's letter, I am doomed as it explains very well my condition and other stuff now he knows me and my conditions.

Yes give a copy to the assessor and send a copy to the DWP office dealing with your claim, marked for the attention of the Decision Maker.
