ESA Appeal was created by jason j
Hi i am near to my ESA appeal date after an adjournment for my medical history, can i ask DWP for the completed PIP factual report form my GP has filled out for me? How do i go about obtaining this. any help thank you.

If there was a GPFR then it should have been supplied in the DWP's submission.

Hi Gordon, i just presumed there would be a GPFR submitted as i was successful with my PIP claim i did ask GP about this form had he filled this in? his answer was he thinks so. No report was found in my latest case bundle.Can i check this before my hearing?

It's not impossible that one was produced by arrived too late to be included in the Decision, the easiest way to find out to check with your GP surgery as there will be a copy on GP records.
