You have a choice, you can write now in the hope of influencing the DM before they make a Decision or you can wait for the Decision and challenge it if you need to.
If you decide to write now then send it to the DWP office dealing with your claim, the address should be on the original letter you received about claiming PIP, failing that, ring the DWP for an address.
You need to highlight the issues you se with he report, stick to ones that are related to you scoring points, so if the assessors spelling is bad, this is not something to raise, but if they have said you can do something and your evidence says you can't then this is. Keep it short, all you are doing is trying to get the DM to read your testimony and evidence in more detail.
