Hello i`m in the process of re-applying PIP for my sister. She came off PIP, in january but gradually shes regressed to shutting everything down and withdrawing socially. Days turn to weeks when she wont get out of bed etc. My question is do i need to fill out new PIP application form or is there a form for re-submitting.

If she closed her claim or did not renew hew previous one then it is a new claim from scratch. She or you on her behalf will have to contact the PIP Claim line to complete a new PIP1 and they will then issue a new PIP2 for her to complete.
