The document you are referring to is not hosted by B&W, you need to contact the site that you downloaded it from in January.
If it is referring to "Seeing" as activity 9 then it is at least six years out of date so I hope that you have not used it for any of your answers then or now in regard your ESA.

No. Not used it as as ESA 50 form sent in May didn't have specific sight question. Hopefully the face to face assessment that was cancelled with 20 mins notice today was because they realised someone with proper qualifications needs to do the assessment due to stroke,brain tumour,sight loss. Hopefully we will be given time explain everyday needs and difficulties . The PIP assessment lady was understand to be fair. So we shall just have to wait for new appointment to arrive. Thank you Gordon. Sorry , but I truly don't remember down loading anything other than here. This is what stress does on top of everything else you have to deal with. My apologises.
