I noticed on the form you can ask for an email version of the form. This would be much simpler for me to fill in is it worth asking for an email copy of the form?
We've not heard of an email'd copy being available, the latest information from the DWP is that there is still not an interactive version of the form, it may just be a copy that you have to print and then fill in.

The likelihood of your conditions changing is a major factor in determining the award length, so it is definitely worth your asking your Consultant whether they will change the letter.
