My wife has just received the fictional novel entitled "Consultation Report Form" after her face to face consultation last month.

The assessors justification for her assessments contain some abbreviations and I haven't got the foggiest idea what they mean. Could someone enlighten me please?

CQ, FH, IO, MSE,SOH.

FE – Further Evidence
FH – Functional History
FRR4 – Telephone contact form
HOC – History of Conditions
IO – Informal Observations
MSE - Mental State Examination or Musculoskeletal Examination
POI – Proof of Identity
SOH – Social & Occupational History
