Use of Care Plans

My GP is updating my care plan for me so I will be able to use a copy with my PIP reassessment.

I noticed both ESA and PIP ask for copies of Care Plans when they specify what to send in wit the form.

I wanted to ask people's experience of Care Plans and if they helped with their claims. Many thanks.
