GP's supporting letter about my mental health-

GP's supporting letter about my mental health- was created by Blackscorpian
Please can anybody advise me if there are any guidelines or a letter template that I could give my GP - I want to ask her to supply me with a supporting letter about how my mental health affects me but I am unsure about what the letter needs to include to be of most use. Can anybody point me in the right direction please?

For PIP there is a GP Notes document

If it is ESA then we don't have any equivalent although you could reasonably easy create one for yourself.

Thank you for all your advice, I'm sure it will help with obtaining a letter of support from my GP to support my ESA claim. Unfortunately not so sure that the DWP will take much notice!
