PIP Award Review Form 1043 was created by Bansheame
My PIP Award Review Form 1043 to arrive by 28.11.17. Hoping to post 23.11.17 any advice on what to send along with my form, apart from medical evidence etc., I read somewhere, (cannot find it info now) to include copy of original claim, copy of HP assessment, copy of award letter.

It depends on how you have answered the questions on the form!

If you had said that your limitations have changed then you need to provide evidence to support this is you can. If you have said that there is no change then it may be to your advantage to send nothing else with the form.

The bottom line is that you have to make a decision about this, the issues are discussed in more detail in the PIP Claim guide.
