hello there
I'm the appointee to my daughter but she has a assessment coming up. And the doctor has wrote in a formal letter she cannot attend because of her Condition. She also has a learning difficulty also little/old information on her.
DO I create new information on her condition etc as
She only just been diagnosed.

Forgive me but I don't understand what you mean by "create new information", have you not completed a PIP2 for your daughter?

You say she has an assessment coming up, has the Assessment Provider seen the GP's letter?
