I went for my PIP medical today and was asked "When you go on holiday where do you go?" I couldn't see how this was relevant to my conditions but answered honestly that I went to Spain. Does anyone know why this question is asked, also whether or not I use a smartphone?

They will be making assumptions of your capabilities from your having gone away, for example how you have been able to manage your limitations when away from home.
You should be able to request a copy of the assessment report by contacting the DWP office dealing with your claim, this should give you a better idea of how the assessor has treated your holiday.
As to the phone, it implies contact with others.
