Hello everyone, I'm trying to figure out what address to send the MR form back to as it's not on the MR form or the original decision letter. Also, they didn't send a medical report wth my decision letter, just the letter on its own, and they haven't contacted my medical references either; they've told me they didn't hear. I've mentioned that in the MR. Any help is really appreciated.
