Including timings for each activity is a suggestion rather than a requirement, I would not do it for the Moving Around activity unless you are able to present considerable empirical evidence as well.
Walking speeds can vary considerably; a healthy person might be able to walk 72m in a minute, someone walking very slowly might only manage 36m per minute, people walking slower than this would be expected to also have problems moving around there house.
Definitely include the times you are away on the form otherwise you can't say that you told them, if you ring then as well then it's likely that they will try and book an appointment while you are on the phone, so be prepared for this.

I will supply the dates that i,d be unavailable for assessment on the form.I,ll think it over whether to phone them also but to be honest would have no issue with them trying to arrange an appointment if I decide yo call..To me it would be a case of getting it over and done with as quick as poss instead of sitting around worrying about it
