Send a copy to the Assessment Provider for your postcode, if you have the address of the actual assessment centre then send it there.
Send a copy to the DWP office dealing with your claim, the address will be on the letter that accompanied the form.
Lastly, take a copy to any assessment and ask the assessor to read it and include it your file before they start the interview.
