Hi im finding it a real struggle to fill the ar1 form
I want to make a appointment with a local person who deals with theses forms
But I cant find anywhere on the form where someone else fills it in for you how do I do this as my handwriting is very poor I have letters detsiling my serve anxiety and eating disorder and says I prefer social isoloation and I have learning problems aswell as not eating well and loads of stuff its to.overwelming as I dont want to send to.much info then send to less info its making me fell more ill and making me worse
