Hi, i have just received my pip review form but it is different from your template. It asks if something has changed and approximately whem.
tell us how you manage this activity now and tell us about any changes you need or help you get from another person.
Is this a new format ? If nothing has changed do i have to fill in the other parts.

Yes, it appears the form was updated about a month ago, the current PIP Claim guide covers the new form.

"tell us how you manage this activity now" and "tell us about any changes to the help you need"
