Hi, I have recently received an ESA application form Capability for Work Questionnaire. I have been in the support group for 5 years. My husband has to fill it in for me but we are not sure if it should be in the first person or the third. Please could you advise?

If he is using your words to complete the form then write in the first person (I) and you sign the form, if he is not using your words then he should write in the third person (she) and you both sign, in either case, explain why you have done what you have.
