A care plan is usually worth sending as it is an independent assessment of your problems and needs and is much more focused on the problems you have in the home than you will likely get from those treating you such as your GP.
Re: sending too much, most people waffle when they write straight to paper, the point that was being made was that revising what you have written can usually significantly reduce the volume but also make it easier to read.
If you are writing the form by hand then the problems that you mention are unavoidable they may also reflect the problems that you have as well.
You are looking at 2-4 weeks to hear whether a face to face is required with an appointment if required 1-2 weeks later, they will book throughout the Christmas period, a Decision will then be 3-5 weeks later.
