Ring or write to the DWP with MR, don't do both as they will treat the call as a request for an MR.

I can see you have lots of problems from your post but you don't explain how these problems limit your ability to complete each of the PIP activities, those who have assessed you lack imagination and they are unlikely to make the connection between you having a wet room and your being unable to use a standard bath or shower, you need to explain these things in detail.

Don't get hung up on the report, the first and most important thing that you need to do is to make sure that you have shown that you meet the criteria for an award. Only when you have done this should you have a go at the report, try and stick to the facts of what happened and to issues that lead to your scoring points, where you can argue against the statements by reference to your form and the evidence that you attached.
