On the whole it is fine but there are a couple of things that I have picked out.

Q1. You have made some non-specific statements, for example;

"with'flare ups', which on average is between 2-5 days a week"

This may be correct but the range does not help you, two days is significantly less than the majority of days and although the five is, the variance undermines it.

again

"The assistance of a specialist peeler and a chair didn't help much"

you need to be specific, you're allowing the assessor to assume that they are of some assistance.

Lastly

"If I did manage to prepare and cook a meal it could take as long as 2+ hours"

It could but maybe it doesn't take you 2 hours.

Don't expect too much from what you have said about your Epilepsy, with no recent seizures it is going to be ignored.

Q2. Just to make sure that you are at best assessing yourself as being unable to reliably walk more than 50m on the majority of days.

30m is actually a very short distance, so expect questions about where you walk and whether it is really this distance, for example; have you measured it.

Thank you Gordon, I realise it was a fairly long post for you to read, very much appreciated.

Your comments have made me think that I really should have engaged with a professional to help me complete the form.

As you say though, I will have an opportunity to explain/expand at my assessment.

Finally, do you think I should add anything else? I have enclosed 10 sheets to address all the activities, slightly worried that is too much?

I had twenty pages so I would not worry about ten :)

As long as you are happy that you have addressed all of the issues that you have with the activities then that is all that you can do, unfortunately, there is no magic solution to these forms its all about doing the work to complete them in sufficient detail.
