The first thing you need to do is to speak to your GP about the form, if they agree that the information they provided was incorrect then they could write a letter correcting it.
You cannot be found fit for Work without your attending a face to face assessment.
It will certainly make it more difficult for you but it does not mean that you cannot receive an award.
