Mandatory reconsideration

I wonder if you could help me please. I phoned the DWP and told them I wanted an MR on my pip decision, I have sent a letter to them also but didn’t realise I needed to send a form in with this letter. Would you kindly let me know where I stand on this matter. Thanks

There is no requirement for you to complete a form, as long as it was clear in your phone call and your letter that you are requesting an MR then it should be sufficient.
