Pip Tribunal was created by MCL
I was previously awarded the lower rate of pip for both however after a review this was changed and I did not score any points I have decided to go to Tribunal after still being refused on a mandatory reconsideration however I am struggling on how to set out my reasons I have COPD,angina, IBS, depression and anxiety, bladder problems I also have heart problems after a heart attack in 2015 I struggle walking, I also don't go out alone due to chest pain, dizziness and toilet problems, I also need motivating to eat and make meals and also to get ready. I just cannot put my point across and would be grateful for any help in how I can do a good appeal

You do not have to provide detailed reasons on the SSCS1 Appeal request form unless you do not intend to provide a written submission later.
There's a worked SSCS1 in the MR & Appeals Submissions document in the PIP area.
Do you have a copy of the assessment report?
A Tribunal Decision will be based on your showing that you meet the criteria for an award so this needs to be the focus of your efforts although you should consider dealing with any issues in the report as a secondary activity.
You also need to consider whether you have properly explained your problems in your claim form, is it possible that you have misunderstood the Descriptors, described your problems in terms that do not match the Descriptors or failed to make appropriate use of the reliability criteria or the need for your limitations to be present on the majority of days to be considered?
