My Daughter who is 30 has been asked to fill in a pip form as her DLA is ending, I am not very hopeful as although she has a lifelong learning disability and struggles with most things she never attends the doctors and is on no medication, I have been told that I can ask for a letter of support from our GP pinpointing the things she struggles with, but really don't know where to start any advice would be appreciated.
Also is there anywhere online where I can view the form so I am prepared before it arrives.

So have a look at our PIP Claim guide for details of the criteria she will need to meet.

The is a GP Notes document in the PIP area that you can give to her GP, they can either complete it or use it as the basis of their own letter.

There's a copy of the PIP2 on the following
