I asked for a copy of the doctors form and my own form 2 weeks before my medical which is Monday. I have not received either. The dwp say they have asked the provider to send the form but this has not been done. How do I officially complain? When I said I wanted to complain the dwp says it's not their fault that atos haven't done it.
