Hi I have a question about the pip 1 form and the initial telephone assessment. I have just done the phone one and I wish now that I had requested the pip1 form as a paper application. The woman in the phone spoke so fast, she didn't clarify and questions I asked, just repeated her answer word for word so that I still didn't understand what she was asking me ( it was about using other evidence they already hold from the dla application and I only now understand this after reading the paper form online)
But the most concerning bit for me is that at the end of the call she asked me whether I had any of the following / schizophrenia , ADHD, Downs, Autism,or dementia .. and she said those so quickly I had a job to follow.
However on the paper form anxiety and depression are listed. She did not ask me about those .. and I do have both. So what should I do if anything ? Thank you for reading.

I would not worry about it, the question is to simply flag on the DWP computer system a claimant at risk of not returning the PIP2. If you struggle with the form then you will be able to get a two-week extension.
