Moving to another town where Universal Credit is?

Hello,
Can I get some advice, please?

If someone who is in the support group and on housing benefit (so not on Un' Credit), moves to a another town in the same county where universal credit is being paid out, and they have to claim housing benefit there, but their chronic health problems have not improved, nor has there been any change in circumstances apart from the actual moving, will they have to be automatically reassessed for ESA/universal credit - or will they simply stay in the support group?

It's not whether you move to a new town or even county but whether you move to a new Local Authority.

If you do then you would need to make a new claim for HB and this will necessitate a claim for Universal Credit unless you are currently receiving or entitled to the Severe Disability Premium. You don't mention an award of DLA or PIP and without a qualifying award of these you won't be eligible for the Premium.
