I ask this question because after visiting physio at the beginning of the year, she began her appointment with an iPad and a script to complete which sounded like the questions on a PIP form, also more recently after speaking to a specialist nurse was told a new updated system has been introduced to them for completion with existing patients and again questions asking how far can you walk etc I don’t mind my records being shared within the NHS but not outside of them.
