I have just subscribed to the site. Hopefully I can get a step by step how to fill in this form.
I have rheumatoid arthritis. Osteopenia. Depression anxiety. Leaking heart valve ( mitral regurgitate valve) high blood pressure. Also currently under the women’s hospital with hydroscopy. I am absolutely petrified how to fill in this form, hope somebody can help me
