Hi,
Good to be a member.
I have just received my PIP form ( I have MS), and having seen from other posts that it is ok to type replies to questions and send them with form.
However, should I staple all the pages together?
Should I then staple the pages to the back of the form?
It's just that I don't want to take the chance of bits getting lost.

My full name is showing, how can I stop it?
The pages need to be easily detachable so that they can be scanned so I would punch a hole through the top left corner of the form and the additional pages and attach them all together using a treasury tag, ribbon or string.
