UC50 Universal credit form

UC50 Universal credit form was created by mk1
I am having trouble filling in the form and have left it and need to do it
I have letter from my doc written problems involving anxiety, leaving the house, adhd, uncontrollable crying episodes and problems with filling in the form.
I have before spiralled into anxiety and depression when getting a job and had crying episodes that make me not be able to hold down a job. along with situations that i dont know when it is going to happen, at the job centre with wrk coach i have cried and they would ask why and i dont know.
I am reffered for a mh assessment, also told my doc about my past suicide attempt that i was hospitalised for but it is written as accidental as i did not want to tell them at the time out of fear.
Is this evidence i can use and the letter from gp?
how best to put this information on the form?
my medication is adhd medication nothing for my anxiety, previously anti depressants.
any help is appreciated.

Is this a reassessment of an existing award or is it a new claim for UC?
Have you looked at our ESA/UC Claim guides?

The information you mention can certainly be used but you need to think about how it is relevant to the Descriptors and Activities. In particular, you should have a look at the section on Substantial Risk as this would seem to be applicable to you.

It is not a new claim for uc but new wca
I have read and got the guides just I still feel confused.
And now I have a letter with an appointment date for assessment and I haven’t returned or completed the form.
Should I still fill it and send it

If you don't complete and return the form then you are relying on your being able to get across your problems with the UC activities at your assessment, whilst you may be able to do this if you are struggling with the form then it would suggest that you will also struggle at the interview.
