Consultation report many mistakes

PA4 took 20 days to arrive and only after phoning twice and putting it in writing
twice.
This was a home assessment.
The report has many mistakes and its to late to do anything about it.
Just a couple of examples below.

1: (MSO not done due to feeling to tired from the assessment)
She actually said (I"m not going to do the MSO as I can see your to tired after the assesment). Many assumptions have been made about the claimant because of this.

For one of the justifications....
2: (they sat down and stood using the sofa), That's impossible because the claimant was seated in a chair the whole time....the assessor was sat on the sofa, there is only one sofa in the room so this is a lie.
How can she assess on mobility, strength etc....?
3: Didn't write all relevant answers given. I was present in the room and took notes.

Question was how can they assess on mobility, strength and fine motor ability without doing the whole assessment?
Plus blatantly lie in the report.
