New PIP Review your award (PIP1043) Form was created by terdun9
Hi I look after my son with serve Mental Heath issues when it comes to Bill,s Paperwork, Local councils, Doctors, and so on. ......
I have just got his PIPS review form to do just looking at it it looks like it is going to be a nightmare to fill in, I'm not sure if this is a new type form or is it an older one are there any notes anywhere that can help at all , also where do I find out the dates of the nights he was at our local hospital being kept in for his own good after having suicidal thoughts, as he lives on the 13th floor of highrise life is worrying at the best of times but he has breakdowns weekly and we are trying to get him moved from there as soon as possible. do we put this on the paperwork as well please help me
I have e-mailed the doctors asking them for a Report Path: local Record and proof of dates of attempts and appointments they said it will cost £25.00 relly!

I am sorry that you are being charged for that information, but it is common practice among doctors. Proof of appointments really won't help you a great deal and in fact the DWP always advises people not to include them. A letter from you stating what you know will be just as helpful.
If he is currently very unwell - see if your doctor will write a letter saying that he either needs a home visit or due to his current poor health would need a paper-based assessment as there is a good chance of his health deteriorating. It might be better for you to pay for that, than the other information.
