Online form for PIP?

Online form for PIP? was created by JeBa
Time has come to fill out yet another form. This time PIP transfer from DLA. I having requested specific reasonable adjustments have discovered the document sent out today has only been partially adjusted. I have been informed that form completion is within 28 days but that does not include the days waiting for the post to arrive. The centre that adjusts documents are known to leave the original date of issuance on the documents and it takes this department significantly longer to send out documents than the normal process.
Is there an online version that can be downloaded?
The PIP helpline people say they will reset the date for return when the new form is sent out by the adjustment centre, but I would rather prepare before the chaos of miscommunication starts.
Your assistance in this matter would be gratefully received.

Hi Jeba

I'm afraid the DWP does not issue PIP as an online form.

You can look at one here - but it's not for sending back

Hello Jeba.

You can request an alternative format PIP2 from

When I received my paper PIP2 I knew I would be unable to manually complete it. I got the address from B&W and contacted the alternative format team, (AF) I put “Time Sensitive PIP2 Alternative Format” in my email subject box to hopefully get someone’s attention. They acknowledged receipt with an email stating they were dealing with a high number of requests, which panicked me a bit. They did contact me to advise that an AF would be sent and with a reasonable adjustment, extended my return date by a further 2 weeks, which I ended up needing. My DLA transitioning PIP2 went in the post, Recorded Delivery this Monday.

My experience using the AF-PIP2 was not an easy one for me, but that just might be because I’m a technophobe, a bright young thing probably would find it easier. As a PDF, Acrobat Reader document I found that I could not navigate easily around it eg. Changing fonts, editing etc. What I did do in the end is, went through it and completed as much as I could such as tick boxes etc. But for each of the main questions, I put in each of the Additional Information boxes ‘Page__Q__ continues over, I then added my own additional sheets, with my name, national insurance number and the page and question number I was answering. I had an additional page for each of the questions on the questionnaire, when I was happy that I had done as much as I could, I printed off the filled PIP2 Questionnaire and incorporated my own answer sheets for each question.

Sorry for the long reply, but I was going to submit something about my AF experience for anyone else that might need the Alternative Format. Hope it helped some.

Sue
