I have just recieved a ESA50 form for completion and obvioulsy my heart rate is pounding and stress levels through the roof, I have found the exceltent letter on the site to request that it is recored, however I do not know if I am supposed to send this with the ESA50 or after I get called for a medical any advice would be apprecaited! many thanks Storm287
