Translation of abbreviations in assessor's report

Have managed to fathom out a few of the many used in the coded report but this one has beaten net searches plus a nurse I asked. CM - can't be case manager cos at the assessor's report stage case manager wouldn't have had any input, and whatever it is assessor attributes to damning me as much as all her other abbreviations.

Only managed to grapple copy of assessor's report, despite asking since Aug, when MP intervened, so deadline well eaten into. Half dead from doing my MR submission,despite lots of help, only so much other people can do, deadline to send Monday to be sure will arrive in time, loads left to do, so quick response would be marvellous thanks.

The following is a list of abbreviations that I have compiled over the years

CM - Current Medication
CQ – Claimant Questionnaire
FE – Further Evidence
FH – Functional History
FME – Further Medical Evidence
FRR4 – Telephone contact form
HOC – History of Conditions
MSE – either Mental State Examination or MusculoSkeletal Examination or Medical Services Examination depending on context
MSK – MusculoSkeletal (probably)
MSO - Medical Services Observations
IO – Informal Observations
OPD – Overwhelming Psychological Distress
POI – Proof of Identity
SOH – Social & Occupational History
