Hi, is it possible to request a copy of the form ESA113 completed by the GP? I failed to return ESA50 for a 12 monthly reassessment on time and an ESA113 form was sent to my local GP practice where it was filled in by a Doctor who doesn't know me (the ones that did know me have left the practice) I have been told I will be getting a face to face interview and it would be useful to know if the information they have is correct.
