I don't know whether to tick no or varied to some questions, if I am off my feet cannot stand or climb two steps 4 out of 7 days which is right?

You don't have to use the tick boxes if they don't allow you to explain your problems adequately, you can mark them "see below" and answer in the comments box but you must answer the question.

Based on your post, you are saying that your limitations vary. You need to explain how they vary and for what period you are affected for each variation.
