I have recently transferred to a different GP practice. In April 2019 I submitted an ESA-50 form, which is still under review by the Centre for Health and Disability Assessments (CHDA). This had my old doctor's details listed. In addition, my previous doctor completed an ESA-113 form on my behalf, which was submitted in May 2019. I am presently receiving ESA at the WRAG rate, as well as PIP.
I would be grateful if you could advise me on who I should contact requesting that my details be updated, as well as what sort of information I should provide (i.e. GP's name, address, telephone, etc.). In addition, would my new GP be required to submit a fresh ESA-113 form, regardless of whether one had been completed previously?
You need to contact both CHDA and the DWP about the new GP, they need the information that you list in your post, you can phone but I would also follow up in writing.
It is possible that a new ESA113 will be issued but the one from May is still relatively new so I would be surprised if they do but it is not impossible,. You might want to contact the GP and ask if they would be willing to see you before they fill in the form so you can answer any questions that they might have about your conditions and limitations.
