Hi I've looked on the site but can't find a template letter to send to DWP requesting a copy oof the HCP Assessment report is there one?
What telephone number is it you ring to request this report?

No, we don't have a letter to request the report you can just ring the DWP office dealing with your claim, the number will be on any letter you have had about the benefit.
If this is for PIP then you can request the report a couple of days after the assessment, if it is for ESA then you will have to wait for the Decision to be made.

Thanks Gordon it's PIP I will call them but I also want to send a letter requesting it to belts and whistles and all that so they can't say I didn't request it.

I phoned for my assessment report today, 6 working days after the assessment. Whilst on the phone she printed it off with a covering letter, put it in an envelope and placed it in the post tray. She said! A very pleasant and helpful lady. I now await receipt.
