I have several chronic physical problems and also a severe anxiety one. One of my physical problems was caused by my behavioural problems many years ago. My question is should I state this on the form?. Do I need to make it clear why I have this particular physical problem? It is very debilitating, but the other problems I have should score me enough points on their own.
Thank you.

You don't need to explain why you have a particular condition but it is not diagnosed then you may need to to add credibility to your limiations.

Yes, just don't overuse it or you risk the effect you are trying to create being diminished.

OK - the particular problem i alluded to is chronic and permanent (I've suffered from it for about 30 years) but I have no written evidence for it from a “professional”. It is absolutely one of the reason why I can't work - should i state the cause for some credibility or or would it be dismissed because it was caused by a mental illness? If asked of course I would explain exactly what caused it.
Thank you in advance for any reply and I wish you all a good Christmas and New Year.

You need to establish the credibility of the condition and its effects, would your GP be prepared to write a letter in support? Even if they do you may want to explain how you acquired the condition to add extra substance, remember, the DWP DM is not medically trained so if the condition is unusual it may help them to understand your problems.
