DLA to PIP home assessment was created by Janice
After first assessor not turning up and no explanation i had a home assessment 2 weeks ago. She was late and stayed 30 minutes. She spent most of it typing on lap top. I have been on DLA for over 20 years. I had a copy of the report and she’s written things that I didn’t say. Had letter from PIP. NO POINTS. Assessor wrote lies and according to her i could do everything on form. She said that i was calm. I could hardly speak and only answered yes and no to most of the questions. She said that i had clean clothes and have. I was was it dressed and had been in same clothes for days and hadn’t combed my hair for days either. She also said that i get dressed every day which i dont and I can walk around a supermarket. I told her I have home deliveries. I don’t know what to do I’m devastated.

I am so sorry this has happened to you.
The first stage to challenging a Decision is for you to request a Mandatory Reconsideration, this needs to be done in writing to the DWP, within one month of the Decision, to the office that dealt with your claim, have a look at our PIP MR & Appeal guide for details of the process, the PIP area also has template letters that you can use to make the request with.

You already have your assessment report so you need to go through this again. Your primary task is to show that you meet the criteria, there are many reasons why you may have failed and you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation;
"the assessor recorded that I walked 50m, I did but they have failed to document that I had to stop every 10m for a rest due to breathlessness"
than their opinions
"based on my observations of the claimant walking I believe that they can reliably walk more than 200m.
Did you send in any recent medical evidence with your claim? If you did, then you also need to quote this in your mandatory reconsideration answers contradicting things that the assessor may have said. The clothing issue comes up quite often and you just need to add the comment just as you have done so above in your answer. The most important thing is to focus on the areas where you think you should have scored points and didn't.
