Hi, one of my illnesses is severe anxiety and depression. I received a text from DFPIP to say that they are sending my review form. This put me severely put me over the edge in terms of panic attacks as I know what now faces me, the form and F2F. Is there any way that I can remove my number off their contact list and add my carers number instead?
If you are unable to speak to them, then you would need to put it in writing and you would need the letter to be signed by both you and your carer. You would have to make it clear what information can be sent to your carer. You also need to request them to remove your number.
