I cant find anything on this site that refers to completing the PIP Award Review Form. I was hoping to find out if I should try to write on the form all the information on the original one which was done 2 years ago, plus any changes. There isn't actually enough space to do it on the form I have been sent now and it will mean adding a lot of additional sheets.
I would advise you to complete the form as you did the original using additional sheets if necessary. The DWP should have enough information to make a decision but in NI(DfC)I found they don't but involve Capita again with a face to face. I hope I am wrong but better to be prepared.
